PAYROLL PROVIDER LETTER IN LIEU OF IRS FORM 941
All companies applying for a CERTS grant must, as part of their application in the CERTS portal, upload a complete copy of their IRS Form 941 for Q1 2020 filed in their own name and TIN.
If a company does not have an IRS Form 941 in its own company name and TIN because it contracts with a third-party to process its payroll and report its payroll taxes, such as with a payroll service provider or a Professional Employer Organization (PEO), then in lieu of the IRS Form 941 the company must upload a completed form letter signed by an authorized representative of its service provider attesting to the company’s total number of employees and total compensation paid for Q1 2020.
